As a new patient with us, we ask that you please arrive 15 minutes prior to your appointment. To make your check-in process as smooth as possible, please have the following items with you:
A driver’s license or other photo ID with your current address.
Digital or hard film copies of your X-rays, MRIs, or any other diagnostic imaging or testing on the extremity we will be treating you for. Copies of these will need to be picked up from the office they were performed at and brought to your appointment with us. These are very important for your initial visit!
Consultation notes from other providers you have seen for this issue, including the reason for that consultation.
A current list of your medications and any allergies to medications.
Your insurance card that we will be billing for your visit. Please, make sure that the insurance company address is also available for our records.
Note: Please, contact your insurance company before your appointment to verify if we are a preferred provider with your plan. If your insurance company requires a referral, this must be obtained PRIOR to your appointment.
