At Proliance Hand, Wrist & Elbow Physicians we want to make scheduling appointments as simple as possible. One of our knowledgeable receptionists will help you contact the physician best trained to meet your specific needs. The fastest way to make an appointment or to change an existing appointment is to call our main numbers:

Registration/Check-in Process

A link to fill out the new patient forms will be emailed or texted to you 3-5 days prior to your scheduled appointment. Please take the time to fill the forms prior to your appointment as this will greatly speed up the check in process and wait time.

Please feel free to contact our office and ask for the administrator if have any concerns or feedback regarding the check-in process.

New Patients

If you have not completed the registration process we ask that you please arrive 30 minutes prior to your appointment. If you have already completed registering, please arrive 5-10 minutes prior to your appointment.

To make your check-in process as smooth as possible, please have the following items with you:

  • A driver’s license or other photo ID with your current address.
  • Digital or hard film copies of your X-rays, MRIs, or any other diagnostic imaging or testing on the extremity we will be treating you for.
    • Copies of these will need to be picked up from the office they were performed at and brought to your appointment with us. These are very important for your initial visit!
  • Consultation notes from other providers you have seen for this issue, including the reason for that consultation.
  • A current list of your medications and any allergies to medications.
  • Your insurance card that we will be billing for your visit. Please, make sure that the insurance company address is also available for our records.
    • Copays are due at time of service.
    • Please contact your insurance company before your appointment to verify if we are a preferred provider with your plan. If your insurance company requires a referral, this must be obtained PRIOR to your appointment.

It is important to obtain these items for to your appointment. If you do not have these items we may have to reschedule your appointment.

If you have a child under the age of 18 years old and you will be authorizing another adult to attend your child’s appointment or authorizing the child to come to the appointment without an adult, please print and complete the form below. PHWE strongly encourages parents and guardians to be part of their minor’s or care.
